Job summary

Cascade Community Healthcare in Washington State is seeking an Intake Clinician III to join our team. This role primarily provides intake assessments (walk-in or scheduled) in compliance with Washington State WAC requirements and Cascade policies. The position may include a small caseload for treatment planning, individual and family services.

Qualifications

• Master’s Degree in counseling or the social sciences required.
• Must be licensed (LMHC, LMFT, LICSW).
• Must be able to pass WA State Patrol background check. A national background check may be required if you have not lived in WA state consecutively the last three years.
• Must comply with agency policies and procedures.
• Must have a valid driver's license.
• Must be able to operate a motor vehicle.
• Must be able to gain access to all types of buildings and residences regardless of accessibility.
• Must maintain current phone number and mailing address on file with HR.

Responsibilities

• Provides intake assessment, primary same-day walk-in assessments, and treatment planning.
• Completes intake assessments within timeframe set by the agency.
• Completes treatment plans within timeframe set forth by the agency.
• Reviews treatment plans goals and short-term objectives with client and records progress in the clinical record.
• Provides individual, family and case management services as appropriate.
• Meets with client as per treatment plan and adjusts plan to ensure appropriate utilization of services.
• Maintains records and reports per policy; completes documentation and data verification.
• Participates in supervision, consultation and training as directed by the Program supervisor.
• Coordinates client data processing procedures and ensures timely reports and communication.
