Job summary

Cascade Community Healthcare in Washington State is seeking an Intake Clinician III to join our team. This role primarily provides intake assessments (walk-in or scheduled) in compliance with Washington State WAC requirements and Cascade policies.   • Must be able to gain access to all types of buildings and residences regardless of accessibility.
  • Must maintain current phone number and mailing address on file with HR.

Responsibilities

  • Provides intake assessment, primary same-day walk-in assessments, and treatment planning.
  • Completes intake assessments within timeframe set by the agency.
  • Completes treatment plans within timeframe set forth by the agency.
  • Reviews treatment plans goals and short-term objectives with client and records progress in the clinical record.
  • Provides individual, family and case management services as appropriate.
  • Meets with client as per treatment plan and adjusts plan to ensure appropriate utilization of services.
  • Maintains records and reports per policy; completes documentation and data verification.
  • Participates in supervision, consultation and training as directed by the Program supervisor.
  • Coordinates client data processing procedures and ensures timely reports and communication.

ABOUT CASCADE:

Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County.

We are looking for an Intake Clinician III to join our team! This position primarily provides Intake Assessments, walk-in or scheduled, according to Washington State WAC requirements and Cascade Policies. This position may have a small caseload providing treatment planning, individual, and family services to assigned clients.

SHIFTS AVAILABLE:

Day Shift: Monday - Friday, 40 hours weekly

PAY SCALE:

Licensed: $34.85 - $42.48 Hourly

WHAT YOU NEED TO GET THE JOB DONE:
  • Master’s Degree in counseling or the social sciences required.
  • Must be Licensed (LMHC, LMFT, LICSW).
  • Must be able to pass WA State Patrol background check. A national background check may be required if have not lived in WA state consecutively the last three years.

WHAT YOU'LL DO:

Provides intake assessment, primary same-day walk-in assessments, and treatment planning.

  • Completes intake assessments within timeframe set for by the agency.
  • Completes treatment plans within timeframe set forth by the agency.
  • Reviews treatment plans goals and short-term objectives with client at each individual treatment session, tracks progress towards meetings those goals, and records client responses in client’s own words in the clinical record.

If applicable, provides individual, family and case management services to assigned clients as appropriate.

  • Meets with client in accordance with expectations outlined in the treatment plan.
  • Ensures proper level of service by reviewing client needs and adjusting treatment plan accordingly to maintain proper utilization of services as evidenced by over/under utilization reports.

Meets direct service productivity requirements as assigned by the program manager/director.

Coordinates treatment services with schools, other relevant agencies, and significant others.

  • Outlines significance of working with primary care providers and other relevant agencies and significant others at intake or time of development of the treatment plan evidenced by treatment plans and progress notes and releases of information.
  • If applicable, periodically encourages client to include the above at various points of treatment when medically necessary or appropriate as evidence in treatment plans and progress notes.

Maintains records and reports in accordance with agency policies and procedures.

  • Completes all required documentation and data verification to be available to MIS according to agency policy and procedure.
  • If applicable, completes treatment plans, crisis plans and progress notes according to agency policy and procedure.
  • Completes other forms as required by agency policy and procedure.

Participates in supervision, consultation, and training at the direction of the Program supervisor.

  • Meet weekly with Program Manager for supervision and to discuss support staff needs.
  • Attend program specific staff meetings.
  • Completes all trainings as assigned within the allotted timeframe set forth by the agency.
  • Participates in quality improvement activities as outlined by the agency.

Coordinate client data processing procedures.

  • Produce required reports and paperwork within timeframe set forth by the agency.
  • Arrange schedule to allow time for coordination of care and return phone calls within one business day (i.e., answer questions for staff related to client data, communication to support integrated care, follow up with client/family, etc.)
