Job summary

Community Solutions is seeking an Intake Clinician to provide strength-based behavioral health assessments and treatment planning for individuals in Santa Clara County, with remote services offered.

The role requires a Master’s in social work or psychology, CA licensure, bilingual preferred, and experience with youth and adults; location in Gilroy, CA with some travel to San Jose; competitive benefits package.

Responsibilities

  • Conduct screening, intake and assessments to evaluate behavioral health needs.
  • Complete Behavioral Health assessments per agency, county, and state standards.
  • Collaborate with client/family to determine needs and goals.
  • Develop individual treatment plans using Transformational Care Planning and coordinate care.
  • Collaborate with treatment teams for clinical consultation.
  • Document timely and comprehensive records per requirements.
  • Maintain charts per Medi-Cal regulations.
  • Develop relationships with community organizations and system providers.
  • Respond to calls and emails within 24 hours on business days.
  • Attend agency meetings and support continuous quality improvement.
  • Use strength-based and solution-focused problem solving.
  • Maintain customer service with internal and external customers.
  • Complete training requirements and apply them to practice.
  • Manage crisis situations and ensure safety.

SUMMARY:

Since its founding as a teen drop-in center in 1972, Community Solutions (CS) has grown in both depth and breadth of services in response to the growing and changing needs in the community. As one of the most comprehensive nonprofit human services agencies in Santa Clara County and the surrounding areas, CS provides prevention, intervention, treatment, and residential services to more than 6,400 clients annually, helping them overcome the challenges posed by mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking.

As our Intake Clinician you will be responsible for utilizing clinical knowledge and best practices to provide assessment of individuals with mental illness and complete strength based, needs driven, culturally relevant behavioral health assessments and admission treatment plan. Services are provided remotely to individuals throughout Santa Clara County.

The Ideal Candidate Will Possess the Following Qualifications and Knowledge:
  • Master’s Degree in social work, psychology or related field plus one-year direct client experience working with mental health issues, drug and alcohol services, psychiatric services, intimate partner abuse issues and/or family systems.
  • Actively registered or licensed with the California Board of Behavioral Sciences.
  • Preferable be bilingual and bicultural.
  • Preferable be licensed as an LCSW, LMFT, or LPCC in the state of California.
  • Knowledge of wellness and recovery concepts, Medi-Cal regulations and program documentation.
  • Knowledge of clinical assessment procedures and ability to successfully obtain and synthesize information on individuals’ presenting problems and history.
  • Knowledge of clinical diagnoses and differential diagnoses.
  • Knowledge of and experience working with culturally diverse individuals.
  • Knowledge of strength-based client driver care.
  • Knowledge and/or certification in evidence-based practices and outcome driven care.
  • Experience working with both youth and adults.
  • Working knowledge of Santa Clara County and surrounding areas community resources.
What You'll Be Doing in the role of Intake Clinician:
  • Utilizes clinical best practices to conduct screening, intake and assessments to evaluate the individual’s behavioral health needs, diagnoses, and medical necessity for services.
  • Completes Behavioral Health assessments consistent with agency, county, and state standards.
  • Collaborates with the individual and/or family to determine the client’s needs and develop treatment goals.
  • Promptly develops individual treatment plans using Transformational Care Planning and is responsible for coordination of care to provide for the client’s stated needs and goals.
  • Collaborates with treatment teams for clinical consultation to ensure a smooth transition from intake to ongoing treatment.
  • Completes timely, clear, and comprehensive documentation in accordance with agency, program, and regulatory requirements.
  • Maintains all charts within Agency and Medi-Cal regulations.
  • Develops and maintains collaborative relationships with appropriate community organizations and system providers.
  • Responds to phone calls and emails within 24 hours on business days unless extenuating circumstance exist. Resolve outstanding issues collaboratively using strength-based solutions focused approach.
  • Attends and participates in Agency meetings and contributes to continuous quality improvement activities to enhance team and program development.
  • Consistently uses strength-based and solution focused problem-solving skills.
  • Maintains good customer service skills with internal and external customers.
  • Successfully completes all training requirements and demonstrates an ability to put trainings into action to ensure agency and contract compliance.
  • Effectively manages crisis situations and maintain safety.

Work Environment and Location:

This position is located in Gilroy, CA with some travel to San Jose, CA.

Special Requirements:

  • License: Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.
