Job summary

Community Solutions for Children, Families, and Individuals in Gilroy, CA is seeking an Intake Clinician to perform screening, intake and assessments, and to develop treatment plans for behavioral health needs. You will coordinate care with families and treatment teams and ensure documentation meets agency, county, and Medi-Cal standards.

Qualified candidates hold a Master’s in social work or related field, actively licensed or registered in California, and have experience with youth and adults.

Qualifications

  • Master’s Degree in social work, psychology or related field with one year direct client experience in mental health, substance use, trauma or related areas.
  • Active or pending licensure with the California Board of Behavioral Sciences.
  • Preferred bilingual/bicultural capabilities.
  • Preferred California licensure (LCSW/LMFT/LPCC) within state.
  • Knowledge of Medi-Cal regulations and documentation practices.
  • Experience with diverse populations and strength-based approaches.
  • Familiarity with Santa Clara County resources.

Responsibilities

  • Conduct screening, intake and assessments to evaluate behavioral health needs and medical necessity for services.
  • Complete Behavioral Health assessments per agency, county and state standards.
  • Collaborate with individuals and families to set treatment goals.
  • Develop individualized treatment plans and coordinate care.
  • Coordinate with treatment teams for clinical consultation.
  • Maintain timely, clear documentation in line with requirements.
  • Maintain charts per Medi-Cal regulations.
  • Build relationships with community organizations and providers.
  • Respond to calls/emails within 24 hours and resolve issues collaboratively.
  • Participate in agency meetings and quality improvement activities.
  • Apply strength-based, solution-focused approaches in practice.
  • Provide excellent customer service to internal and external clients.
  • Complete required trainings and implement learnings to ensure compliance.
  • Respond effectively to crises and maintain safety.

SUMMARY

Since its founding as a teen drop-in center in 1972, Community Solutions (CS) has grown in both depth and breadth of services in response to the growing and changing needs in the community. As one of the most comprehensive nonprofit human services agencies in Santa Clara County and the surrounding areas, CS provides prevention, intervention, treatment, and residential services to more than 6,400 clients annually, helping them overcome the challenges posed by mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking.

  • Master’s Degree in social work, psychology or related field plus one-year direct client experience working with mental health issues, drug and alcohol services, psychiatric services, intimate partner abuse issues and/or family systems.
  • Actively registered or licensed with the California Board of Behavioral Sciences.
  • Preferable be bilingual and bicultural.
  • Preferable be licensed as an LCSW, LMFT, or LPCC in the state of California.
  • Knowledge of wellness and recovery concepts, Medi-Cal regulations and program documentation.
  • Knowledge of clinical assessment procedures and ability to successfully obtain and synthesize information on individuals’ presenting problems and history.
  • Knowledge of clinical diagnoses and differential diagnoses.
  • Knowledge of and experience working with culturally diverse individuals.
  • Knowledge of strength-based client driver care.
  • Knowledge and/or certification in evidence-based practices and outcome driven care.
  • Experience working with both youth and adults.
  • Working knowledge of Santa Clara County and surrounding areas community resources.
What You'll Be Doing in the role of Intake Clinician
  • Utilizes clinical best practices to conduct screening, intake and assessments to evaluate the individual’s behavioral health needs, diagnoses, and medical necessity for services.
  • Completes Behavioral Health assessments consistent with agency, county, and state standards.
  • Collaborates with the individual and/or family to determine the client’s needs and develop treatment goals.
  • Promptly develops individual treatment plans using Transformational Care Planning and is responsible for coordination of care to provide for the client’s stated needs and goals.
  • Collaborates with treatment teams for clinical consultation to ensure a smooth transition from intake to ongoing treatment.
  • Completes timely, clear, and comprehensive documentation in accordance with agency, program, and regulatory requirements.
  • Maintains all charts within Agency and Medi-Cal regulations.
  • Develops and maintains collaborative relationships with appropriate community organizations and system providers.
  • Responds to phone calls and emails within 24 hours on business days unless extenuating circumstance exist. Resolve outstanding issues collaboratively using strength-based solutions focused approach.
  • Attends and participates in Agency meetings and contributes to continuous quality improvement activities to enhance team and program development.
  • Consistently uses strength-based and solution focused problem-solving skills.
  • Maintains good customer service skills with internal and external customers.
  • Successfully completes all training requirements and demonstrates an ability to put trainings into action to ensure agency and contract compliance.
  • Effectively manages crisis situations and maintain safety.

Work Environment and Location

This position is located in Gilroy, CA with some travel to San Jose, CA.

Special Requirements
  • Must be able to meet and receive a criminal records clearance as required by specific programs, licensing regulations and Community Solutions practices.
  • License: Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.
