Community Solutions is seeking a Clinical Coordinator to provide clinical consultation, client care direction, and staff coordination across Santa Clara and Monterey Counties. POSITION TITLE: Clinical Coordinator

SUMMARY

Since its founding as a teen drop-in center in 1972, Community Solutions (CS) has grown in both depth and breadth of services in response to the growing and changing needs in the community. As one of the most comprehensive nonprofit human services agencies in Santa Clara County and the surrounding areas, CS provides prevention, intervention, treatment, and residential services to more than 6,400 clients annually, helping them overcome the challenges posed by mental health issues, substance abuse, trauma, severe family dysfunction, sexual and domestic violence, and human trafficking.

As our Clinical Coordinator, you will provide: clinical consultation and direction of client care and referrals to the staff; develop and maintain competent, quality client services; and, well-coordinated education efforts as required by contracts. Services are provided throughout Santa Clara County and San Benito County.

This full-time opportunity is available for immediate hire within Santa Clara and Monterey Counties – California. Local candidates are highly preferred, as this opportunity requires a hybrid schedule and travel within a commutable distance to either to our Gilroy Headquarters, Morgan Hill or San Jose offices based upon proximity to the clients you will be serving.

Educational Qualifications

Must possess a master’s degree in the mental health profession and a minimum of two (2) years’ experience in a related field required. LCSW/MFT preferred. Licensure must be active and in good standings with the California Board of Behavioral Sciences (BBS).

License

Registered as a Marriage Family Therapist Intern (MFT) or Associate Clinical Social Worker (ACSW) through California Board of Behavioral Science (BBS). Required to maintain current registration and remain active and in good standing with the BBS. Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.

Knowledge Of
  • Aptitude to accurately apply Medi-Cal regulations and documentation requirements based on training and ensure audit ready charts.
  • Cultural competency by effectively working with people differing in value systems, ethnicity, cultural backgrounds, language capabilities and disabilities.
  • Clinical knowledge and expertise within scope of practice in group, family and individual therapy.
  • Empowerment, advocacy and support techniques for victims/survivors of violence, people with mental illness or substance abuse issues.
  • Community resources in Santa Clara County and surrounding areas.
Ability To
  • Demonstrate leadership with emphasis on taking initiative, advocating for the client, promoting teamwork and developing the staff to expand their capabilities and encourage them to learn new skills.
  • Maintain assigned case load, achieve productivity expectations based on program contract and/or program needs.
  • Effectively engages and interacts with clients and families demonstrating strong interpersonal skills. Handles all client interactions in a courteous and professional manner using effective reflective listening skills and seeks to understand others divergent views.
What You'll Be Doing in the role of Clinical Coordinator includes
  • Providing leadership and program coordination to ensure the needs of clients are met and services meet contract and/or certification requirements.
  • Coordinating staff and the program’s clinical activities and reports/updates Program Manager and Coordinators on status.
  • Assigning schedules and monitors caseloads in partnership with managers, for staff, interns and volunteers.
  • Reviewing charts on a weekly bases with staff to ensure audit ready charts. Supports audit practices for the program.
  • Assisting paraprofessional staff with development and implementation Transformation Care Plans. Responsible for coordination for clinical care to provide for clients’ stated needs and goals.
  • Supervising the clinical skills development of interns, trainees and waivered employees. Conducts initial orientation, training and development. Responsible for all required documentation and reporting required by the academic programs and the BBS process as assigned by Program Manager.
  • Enforcing all Agency protocols, including Medi-Cal and non Medi-Cal procedures. Provides feedback on response forms to referral sources, follows up on pending referrals, ensures timelines are met and referrals are opened and discharged within program timelines.
  • Maintaining program statistics, oversees program Utilization Review (UR) process, review and audit charts, monitors chart documentation timelines, tracks and evaluates program outcomes and submits progress reports in a timely manner, as assigned.
  • Consistently using strength-based and solution focused problem-solving skills.
  • Maintaining excellent customer service skills with internal and external customers.
  • Leading regular meetings with program staff and/or volunteers in order to train, schedule services, plan, anticipate and overcome obstacles and share information as assigned.
  • Providing direct services, as needed. Provides back-up crisis response coverage for evening and after hours.
  • Conducting screening, intake and assessments and carries a caseload, as assigned. Assesses clients’ needs and goals to determine appropriate resources, including the referral to professional services without delay.
  • Completing detailed clinical records and other required documentation in accordance with agency, program and regulatory requirements. Reviews and sign off on clinical documentation within Agency timelines for paraprofessional staff, and other staff, as assigned.
  • Participating in on-call and after-hours rotation, as needed. Flexible schedule may be required to work some evenings and weekends.
