Job summary

Haven Hospice in Orange Park, FL, seeks an RN Clinical Supervisor to oversee hospice staff, coordinate services, and guide clinical decisions with a strong emphasis on quality and compassionate end-of-life care.

The ideal candidate will be a clear communicator, team leader, and dependable role model, dedicated to clinical excellence, documentation, and patient-centered planning.

Qualifications

• Registered Nurse with current, unencumbered licensure to practice professional nursing in the state.
• BSN preferred.
• Minimum of two years of supervisory or management experience preferred.
• Current CPR certification and active driver's license with insured vehicle.

Responsibilities

• Review new patient referrals and determine care needs; assign appropriate hospice staff.
• Support and guide clinical staff through home visits, case reviews and real-time consultations.
• Help develop patient care plans, set treatment goals, and prioritize care.
• Lead and participate in team conferences to ensure coordinated, holistic care.
• Conduct regular chart audits and provide feedback for quality improvement.
• Participate in hiring, onboarding and training of clinical team members.
• Support staff development and continuing education programs.
• Assist in policy development and uphold professional care standards.
• Engage in community outreach and promote hospice services.
• Contribute to quality improvement initiatives and organizational goals.

Qualifications

• Registered Nurse with current, unencumbered licensure to practice professional nursing in the state of operation.
• Graduate of an accredited school of nursing; BSN preferred.
• Previous experience in hospice or home care setting required.
• Minimum of two (2) years of management or supervisory experience preferred.
• Current CPR certification, maintained in accordance with organizational policy.
• Must be a licensed driver with an automobile that is insured per state and/or organizational requirements and is in good working order.
• Proven ability to work effectively within an interdisciplinary team setting.
• Demonstrates excellent observation, nursing judgment, and communication skills-both verbal and written.
• Understands the hospice philosophy and the emotional, clinical, and ethical considerations surrounding end-of-life care.
• Knowledge of pain and symptom management principles.Adheres to accepted professional standards and practices.
• Commitment to clinical excellence and thorough documentation.
