We are hiring an Outreach Coordinator for the Chautauqua, Cattaraugus, and Wyoming areas. The role involves identifying potential clients, engaging with clients and families in the community and their homes, and making appropriate medical/social referrals with all necessary documentation.
Key Responsibilities:
- Develop and maintain relationships with community referral sources such as hospitals, doctors' offices, nursing homes, and community service agencies.
- Serve as a liaison to ensure referral source satisfaction and needs assessment.
- Attend community events, coalition meetings, and take leadership roles in community boards.
- Promote the company within targeted communities and audiences.
- Other duties as assigned.
Qualifications:
- Bachelor's Degree or Associate's Degree with 2 years of experience in Human Services and Marketing/Sales.
- Experience working with Medicaid is mandatory; knowledge of Medicaid Waiver Programs is preferred.
- Minimum of 2-3 years healthcare experience in marketing and servicing referral sources.
- Strong presentation, organizational, communication, and interpersonal skills.
- Knowledge of healthcare applications in home care.
- Ability to travel up to 80%; coverage includes the greater Cattaraugus, Chautauqua, and Wyoming areas, based in Jamestown, NY.
Additional Skills:
- Customer service and interpersonal skills, professionalism, and team cooperation.
- Proficiency in Word, Excel, data manipulation, and systems development.
- Critical thinking and multidisciplinary understanding of healthcare.
Physical Demands and Work Environment:
- Light work, exerting up to 20 pounds occasionally.
- Office and community settings.
Compensation and Benefits:
- $28.85/hour, full-time, 40 hours/week.

Schedule:
- 8-hour day shifts, morning shifts.
Experience Required:
- Human Services/Marketing: 2 years.
- Medicaid Waiver: 1 year (preferred).
- Healthcare service referrals: 2 years (preferred).
