Full-time LPN Nurse - Intake/Admissions

This position includes 12-hour shifts, either 7a-7p or 7p-7a, with rotating weekends.

Job Summary

The Admission LPN is responsible for coordinating hospital admissions through communication and relationships with referral sources, providers, and intake staff.

Essential Functions

  1. Reviews referral information for medical necessity, level of care criteria, and medical stability.
  2. Assists in presenting information to providers for admission decisions.
  3. Reviews and enters medication information for admitted patients.
  4. Begins the admission process, including skin assessment and nursing assessment.
  5. May take admission orders from providers.
  6. Manages and records patient’s home medications.
  7. Keeps families and referral sources informed of decisions.
  8. Recommends additional resources when appropriate.
  9. Accompanies patients to the unit and introduces them to staff.
  10. Provides thorough handoff to the receiving nurse.
  11. Demonstrates knowledge of regulatory agencies, insurance companies, and compliance standards.
  12. Completes monthly reports and communicates effectively with management.
  13. Operates ethically with referral sources, patients, families, and external agencies.
  14. Maintains required competencies and follows departmental policies and procedures.
  15. Maintains good attendance, professional appearance, and punctuality.
  16. Participates in outcome evaluation, quality improvement, and utilization review.
  17. Provides feedback for performance improvement and system monitoring.
  18. Identifies and reports variables affecting service quality.
  19. Participates in hospital quality improvement initiatives.

  • Performs additional tasks as assigned.

Education, Training, and Experience Required

  • Education: Licensed Practical Nurse (LPN) with behavioral health experience.
  • Experience: Preferably one year in a similar role or clinical experience with the primary population served. Behavioral health admissions, utilization review, and insurance experience are required.
