Intake / Admissions LPN Nurse-Night Shift

This full-time position involves 12-hour shifts, either 7a-7p or 7p-7a, with rotating weekends.

Job Summary

The Admission LPN is responsible for coordinating hospital admissions through communication with referral sources, providers, and intake staff.

Essential Functions
  • Review referrals for medical necessity, level of care, and stability.
  • Assist in presenting information for admission decisions.
  • Review and enter medication information for admitted patients.
  • Begin the admission process, including skin and nursing assessments.
  • Take admission orders from providers as needed.
  • Manage patient's home medications and keep families and referral sources informed.
  • Recommend additional resources when appropriate.
  • Escort patients to the unit and introduce them to staff.
  • Provide thorough handoffs to receiving nurses.
  • Maintain knowledge of regulatory, insurance, and agency standards.
  • Complete reports and communicate effectively with management.
  • Operate ethically with referral sources, patients, and external agencies.
  • Maintain competencies and adhere to departmental policies.
  • Ensure good attendance, professional appearance, and punctuality.
  • Participate in quality improvement and outcome evaluations.
  • Identify and report variables affecting service quality.
Qualifications
  • Licensed Practical Nurse (LPN) with behavioral health experience.
  • Preferably one year of relevant experience, including behavioral health admissions and insurance/utilization review.
