Qualifications

• BSN Required
• Graduate of a WOCNCB accredited education program with current Wound / Ostomy /Continence certification preferred
• Exceptions to the educational requirement will be evaluated on an individual basis. A strong candidate in the process of completing the educational requirement will be considered
• Recent acute care experience preferred. Weekend coverage required

Responsibilities

• Provides direct care to patients with complex wounds, ostomy care and continence needs.
• Collaborates with the health care team to develop and implement care plans.
• Educates patients, families, and staff on wound and ostomy care and management.
• Evaluates patient response to treatment and adjusts care plans accordingly.
• Uses protocols and evidence-based guidelines for therapy and equipment.
• Participates in orientation, training, and quality improvement initiatives.
• Documents care accurately in the electronic medical record.
• Collaborates with the Inpatient WOCN Team at Hartford Hospital.
