Job summary

Qualifications

• Conduct psychosocial assessments of Veterans in collaboration with the interdisciplinary team, the Veteran, family members, caregivers, and/or significant others.

Responsibilities

• Provide discharge planning and crisis intervention for Veterans on acute medicine, surgery, neurology and ICU units.
• Deliver individual, group, and family counseling as part of brief evidence-based therapies.
• Coordinate hand-offs and referrals to PACT or Specialty Clinic for follow-up after discharge.
• Document interventions in the electronic medical record and liaise with community agencies.
