Responsibilities

• Coordinates patient care, facilitates group therapy and develops programming for identified patients.
• Establishes and maintains positive relationships with patients, physicians and referral sources.
• Plans and facilitates daily adult groups including educational and psychotherapy groups.
• Documents in Electronic Medical Records within specified time frames.
• Attends staff meetings and participates in treatment planning and discharge processes.
