Qualifications

• Minimum Qualifications: Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred; or score a minimum of 40 points on Pharmacy's Competency Based Advancement worksheet.

Responsibilities

• Coordinate all aspects of patients' pharmaceutical needs in patient care areas; medication order approval and distribution with appropriate dosing; review patient medication profiles for interactions, duplication, and allergies; collaborate with physicians and nurses to ensure optimal therapy; supervise Pharm.D. students, technicians, and provide educational programs for hospital staff.
