• Licensed as a social worker in Ohio.
• Ability to identify and plan for resolution of psycho social problems; gather/interpret data from multiple sources.
• Maintain discretion and provide counseling in emotionally-charged situations; communicate orally and in writing; use modern technology.

Responsibilities

• Conduct assessments and provide social psychotherapy, counseling and psychosocial intervention with patients and families.
• Provide case management across the continuum of care, including discharge planning.
• Record assessment, interventions, progress in the medical record.
• Refer patients to community resources, contacts with various community agencies.
