This part-time, on-site position supports a full continuum of care coordination across the patient journey, from admission through discharge, collaborating with physicians, families, and external providers.

The role emphasizes clinical assessments, care planning, education, and resource coordination to promote quality outcomes and efficient care transitions in a dedicated hospital setting.

Qualifications

• Graduate of an accredited school of nursing and licensed in Ohio.
• 2 years of clinical experience in an acute care setting.
• Knowledge of evidence-based practice and disease management protocols.
• Excellent verbal and written communication skills.
• Utilization review and/or discharge planning experience preferred.
• Critical care/emergency department experience highly desired.

Responsibilities

• Determine appropriate level of care on admission using hospital criteria.
• Perform patient and caregiver assessments to identify problems/needs.
• Organize assessment data into an individualized plan of care with outcomes.
• Determine continued stay status.
• Participate in multidisciplinary rounds and patient education.
• Track and manage potential avoidable hospital days.
• Provide clinical information to external providers and managed care organizations.
• Assist in appeals and denials management.
• Conduct comprehensive clinical and psychosocial assessments to identify needs.
• Identify barriers to care and connect patients with community resources.
