Job summary

HealthPro Heritage is seeking a compassionate Physical Therapist for an inpatient SNF setting in Silver Spring, MD. This full-time role offers a diverse caseload, strong clinical support, and opportunities for professional growth within a collaborative, team-centered culture.

You will assess residents, develop individualized treatment plans, supervise assistants, and coordinate care across nursing staff and families to maximize functional outcomes.

Qualifications

• Bachelor or Master of Science degree in Physical Therapy.
• Current license in the state of practice.
• Experience in inpatient SNF settings preferred.
• Excellent oral and written communication skills.
• Ability to develop and implement a plan of care.
• Knowledge of OBRA, HIPAA and reimbursement guidelines.
• Ability to lift up to 50 lbs and perform required physical tasks.

Responsibilities

• Review available resident information to assess impact on treatment.
• Perform thorough functional assessment using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Document evaluation, treatment and progress notes timely and accurately.
• Instruct Assistants in treatment procedures and supervise staff.
• Enter a progress note in the resident record after each session.
• Revise treatment plans as needed throughout the treatment process.
• Document the discharge summary using prescribed formats.
• Orient residents to services and inform of potential risks.
• Collaborate with nursing staff and families to maximize function.
• Instruct residents and families on home exercise programs.
• Assist in referrals after discharge and stay updated on community resources.
• Attend rehabilitation team conferences and department meetings.
• Pursue continuing education and participate in quality improvement.
