Job summary

Northshore Home Health Care is seeking a Rehabilitation Registered Nurse to provide specialized inpatient rehab care in a mission-driven, patient-centered setting. You will assess, plan, and implement evidence-based nursing care for patients recovering from stroke, brain and spinal cord injuries, and other complex conditions.

Join an interdisciplinary team to deliver goal-oriented interventions, educate patients and families, and support safe discharge planning and transitions of care.

Qualifications

• Registered Nurse licensure and strong clinical judgment.
• Experience or aptitude in rehabilitation care for complex neurologic and medical needs.
• Ability to work effectively within an interdisciplinary team.
• Strong communication and patient/family education skills.
• Commitment to evidence-based practice, safety, and patient-centered care.
