Job summary

• Administer medications and perform required nursing interventions.
• Monitor and document patient vitals and overall clinical status.
• Assist with activities of daily living (ADLs) and support comfort and wellbeing.
• Reinforce therapy plans and collaborate with the care team to support rehabilitation goals.
• Coordinate and communicate effectively with RNs, therapists, and physicians.

Qualifications & Skills

• Current LPN/LVN licensure.
