Community Rehabilitation Hospital West, operated with Lifepoint Health, is seeking a Speech-Language Pathologist to join our team in Brownsburg, Indiana. You will assess patients, establish and implement plans of care, and coordinate with physicians to meet rehabilitation goals.

Qualifications

• Master's degree in Speech-Language Pathology.
• Experience in acute or rehab setting preferred.
• Current SLP license and ASHA certification.
• CPR certification required.

Responsibilities

• Performs patient assessment.
• Establish a plan of care.
• Provides care in accordance with physician's orders and plan of care.
• Assesses treatment effectiveness and modifies plan to achieve goals.
• Plans for patient discharge and communicates with patient and care team.
• Participates in performance improvement and program development.
