Job summary

Sanford Health is recruiting an RN to provide individualized nursing care using the nursing process across the continuum of care. You will collaborate with physicians and a multidisciplinary team to plan, implement and evaluate patient care while focusing on education and discharge planning.

The role emphasizes patient assessment, triage and coordination of care, with a strong commitment to quality and standards of practice. Bachelor’s degree preferred and an active RN license are required.

Qualifications

• Graduate from an accredited nursing program required, including AACN/ACEN/NLN CNEA.
• Bachelor’s Degree in nursing preferred.
• Currently holds an unencumbered RN license; multistate licensure may apply in NLC states.

Responsibilities

• Provide individualized nursing care using the nursing process: assessment, diagnosis, outcomes planning, implementation and evaluation.
• Collaborates with physicians and inter-professional colleagues to plan, implement and evaluate care.
• Perform patient education, triage and coordination of care across all phases of the patient journey.
