Job summary

Exalt Health Rehabilitation Hospital WP in Florida is seeking a dedicated Physical Therapist to join our interdisciplinary team. You will perform patient evaluations, design individualized rehab plans, and provide skilled interventions to enhance functional outcomes for patients recovering from illness and injury.

You will educate families, document sessions, and collaborate with physicians, nurses, and social workers to ensure seamless transitions from hospital to home.

Qualifications

• DPT degree from an accredited program.
• 2 years inpatient rehabilitation experience preferred.
• Current PT license in the state of practice.
• Current BLS certification.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct comprehensive patient evaluations including ROM, gait, strength, and balance.
• Develop and implement individualized treatment plans to maximize independence.
• Deliver manual therapy, therapeutic exercises, gait training, and modalities.
• Educate patients and families on home programs and device use.
• Monitor progress, adjust plans, and document sessions accurately.
• Maintain meticulous patient records and documentation.
• Participate in interdisciplinary team meetings with nurses and social workers.
• Maintain a professional, supportive care environment.
