Job summary

Highpoint Health Winchester with Ascension Saint Thomas is seeking a Rehabilitation LPN for our 12-bed inpatient rehab unit. This part-time day position supports a 2x12 schedule (7a-7p) with rotating weekends and a strong interdisciplinary team to deliver patient-centered care.

You will provide direct nursing care under supervision, assist in assessments, and help with plan of care, daily living activities, and documenting care.

Qualifications

• Active Tennessee LPN license (single-state or compact).
• Graduate of a Practical Nursing program.
• BLS certification within 30 days of hire.

Responsibilities

• Provide direct patient care under supervision per standards.
• Assist in assessment data collection for RN review.
• Carry out plan of care interventions and document care.
• Assist with activities of daily living and mobility.
• Monitor patient status and communicate with RN/physician/families.
