Job summary

Lifepoint Health is seeking a Rehabilitation LPN to join the Winchester team for PRN night shifts. You will provide bedside nursing care under RN supervision on a 12-bed inpatient rehab unit, supporting daily living activities and wound care.

Responsibilities include data collection for assessments, administering medications as allowed, and collaborating with therapy, physicians and case management to ensure patient-centered care. Tennessee LPN licensure is required.

Qualifications

• Current Tennessee or compact state LPN license.
• Graduate of a Practical Nursing program.
• BLS certification within 30 days of hire.
• One year of previous experience preferred.

Responsibilities

• Under supervision and delegation by a registered nurse, provides direct patient care per scope and standards.
• Under RN direction, assists in patient assessment by collecting data for RN review.
• Performs plan of care interventions including wound care and specimen collection; may administer medication; documents care.
• Assists RN with development of plan of care.
• Assists patients with activities of daily living (hygiene, mobility, nutrition).
• Monitors and communicates patient status and responses to interventions with RN, physician, patient and families.
• Advocates for patients' physical and emotional well-being.
