Job summary

HCA Florida Bayonet Point Hospital in Hudson, FL is seeking a PRN Clinical Rehabilitation Specialist to help identify candidates for inpatient rehab and coordinate timely patient transfers. You will work with the Rehab Medical Director to ensure appropriate placement and serve as rehab expert across referral sources.

The role focuses on intake, admission screening, nursing coordination, family education, and liaison with case managers and payors.

Qualifications

• 2 years of experience in rehab or case management.
• Basic Cardiac Life Support within 30 days of start date.
• Licensed Physical Therapist, or OT, or RN, or Speech Therapist.

Responsibilities

• Assist and coordinate intake and pre-admission screening processes.
• Perform on-site and off-site clinical evaluations to determine admission eligibility.
• Provide information to patient/family about rehab and program features; conduct tours of the rehab unit.
• Obtain information on patient resources and insurance benefits.
• Demonstrate rehab admission criteria to identify qualifying patients.
