Job summary

Qualifications

• Experience with grief, trauma, and adjustment issues preferred.

Responsibilities

• Develops, implements and evaluates patient care according to identified needs.
• Writes appropriate treatment plans according to patient diagnosis, age and clinical presentation; discharge plans and termination process.
• Collaborates with guardians and community agencies to insure effective service delivery.
• Completes patient care assignments with accurate documentation in the medical record and timely billing information.
• Designs and delivers effective patient education.
• Maintains open communication with the healthcare team to support patient outcomes.
