Job summary

Rochester Regional Health in Clifton Springs, NY is seeking a Care Navigator for the Inpatient Psychiatric department. This 24-hour-per-week role offers day or night shifts with every other weekend rotation.

You will assist Unit Leadership in assessing patient functioning and in developing and implementing treatment plans as needed. You will provide direct care, participate in multidisciplinary teams, document patient responses, advocate for access to services, and engage in quality improvement

Qualifications

• Minimum of 2 years in human services/medical/customer service, or associate degree (bachelor’s preferred).

Responsibilities

• Provides direct care to patients individually and in groups, including activities of daily living and psychiatric rehab.
• Participates in developing a comprehensive treatment plan with patient input.
• Documents patient responses to treatment timely and accurately.
• Advocates for access to required services and participates in quality improvement to reduce costs.
• Contributes to a multidisciplinary team with feedback and professional collaboration.
