Qualifications

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in State of Illinois
• BLS from American Heart Association upon hire
• Masters or Doctoral Degree in Physical Therapy (Preferred)

Responsibilities

• Demonstrates NM values and provides direct patient care.
• Maintains patient plans of care with collaboration.
• Completes timely documentation in EMR and progress notes.
• Provides education and supports continuous improvement within department.
