You will develop evidence-based plans of care to enhance mobility, independence, and safe discharge readiness while managing complex cases. Collaborating with physicians, nurses, and interdisciplinary teams, you will document assessments, interventions, and outcomes in the EMR, contribute to teaching, research, and quality
