Job summary

Talented Medical Solutions is seeking an experienced Hospital Inpatient Pharmacist to join our acute care pharmacy team on a variable shift schedule. The ideal candidate will have recent inpatient hospital experience, Epic proficiency, and the ability to provide safe, evidence-based pharmaceutical care across all patient populations, including pediatric and neonatal patients.

Qualifications

• Active Pharmacist license in good standing is required.
• Graduate of ACPE-accredited School of Pharmacy (PharmD preferred).
• Recent inpatient hospital pharmacy experience is required.
• Current Epic experience is mandatory.
• Must successfully pass IV competency/training.
• Strong knowledge of sterile compounding and IV medication preparation.
• Excellent communication, critical thinking, and clinical decision-making skills.

Responsibilities

• Verify and dispense medications accurately in an inpatient hospital setting.
• Review medication orders for appropriateness, dosing, interactions, allergies, and efficacy.
• Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient care.
• Prepare, review, and supervise sterile compounding and IV medication preparation.
• Provide clinical pharmacy services for adult, pediatric, and neonatal patients.
• Monitor patient medication therapy and recommend interventions as appropriate.
• Ensure compliance with hospital policies, state and federal regulations, and accreditation standards.
• Document clinical interventions and medication-related activities in the EMR.
