Qualifications

• Requires Bachelor's degree in Pharmacy; PharmD preferred.
• Ohio State Board of Pharmacy license is required.
• 5–7 years of relevant pharmacy experience preferred.
• Experience supervising is not required.

Responsibilities

• Contacts and confers with physicians and nursing staff to resolve drug therapy questions; documents resolutions.
• Reviews new medication orders against patient profile for completeness, allergies, dose reasonableness, duplicates, and reactions.
• Keeps all required records including controlled substances.
• Provides care for patients across age groups; supports safety and education.
• Performs other duties as required.
