Kaiser Permanente’s Fremont Hospital inpatient pharmacy is seeking a Pharmacist to consult with patients and medical staff on medication therapy, dispense and prepare pharmacy products, and ensure accurate documentation.

The role emphasizes collaboration with healthcare teams, review of lab results, and ensuring safe, effective drug therapy while maintaining compliance with policies and procedures.

Formación

• B.S. degree in Pharmacy OR Pharm.D required.
• Pharmacist license in California within 1 month of hire.
• Basic Life Support
• National Provider Identifier required at hire.

Responsabilidades

• Consult with patients and medical personnel regarding medication therapy.
• Monitor and manage medication therapy for hospitalized or ambulatory patients.
• Provide medication information and patient education on use, storage, and precautions.
• Order and adjust dosages and coordinate drug therapy with lab results.
• Supervise and educate Pharmacist Residents, Interns, and Technicians.
