MD Anderson Cancer Center in Houston seeks a licensed pharmacist to join the inpatient/outpatient pharmacy team, providing expert medication management in an ambulatory oncology setting. The ideal candidate holds a Pharm.D. (or BS in Pharmacy) and brings strong clinical knowledge with teamwork and precise communication.

• Licensed pharmacist with Pharm.D. or Bachelor's degree in Pharmacy from an accredited college.
• Strong clinical knowledge and commitment to patient-centered care in an ambulatory oncology setting.
• Hospital pharmacy experience (6 months or more) preferred; excels in accuracy, communication and collaboration.

Responsabilidades

• Reviews, clarifies, and processes medication orders accurately and in a timely manner.
• Ensures appropriateness of ordered medications considering disease state, BSA, age, height/weight, labs and renal function.
• Utilizes clinical information systems to support pharmaceutical care: medication order entry, verification, distribution, checking, reconciliation.
• Performs and documents therapeutic interventions and participates in event reporting.
