RWJBarnabas Health’s Clara Maass Medical Center in Belleville, NJ, is seeking an Inpatient Palliative Care Nurse Practitioner or Physician Assistant. You will deliver high‑touch, patient‑centered palliative care across the hospital continuum.

Responsibilities include managing complex symptoms, leading goals‑of‑care discussions, and coordinating with physicians, social work, and care teams to ensure seamless transitions and compliant documentation.

Qualifications

• Active New Jersey NP or PA license with national board certification.
• DEA and CDS registration, or ability to obtain by start date.
• Minimum 2 years of palliative care experience, with Palliative Care specialty certification preferred.
• Strong communication and comfort working within interdisciplinary teams.
• Ability to perform at the top of your license with holistic care.

Responsibilities

• Provide high‑touch, patient‑centered, evidence‑based palliative care across the hospital continuum, from high‑acuity ED/ICU to ongoing management on units.
• Manage complex symptoms for patients with advanced illness and chronic disease.
• Lead goals‑of‑care and advance care planning conversations with patients and families, coordinating with partner SNFs/hospices when appropriate.
• Integrate into interdisciplinary hospital workflows, partnering with physicians, APPs, social work, care coordination, and operations staff.
• Complete timely documentation consistent with clinical, billing, and quality standards.
• Balance discharge planning with intake to manage transitions, honoring patient preferences.
