Job summary

Texas Health Physicians Group in Fort Worth, TX is seeking a Full-Time Palliative Care Nurse Practitioner to join a dedicated team providing comprehensive palliative care to patients and families facing serious illness.

The NP will focus on inpatient care, require TX NP license and certification, and bring 1–2 years of palliative care experience. EPIC is used for documentation, and a strong emphasis on communication with care teams is essential.

Qualifications

• Must be graduate of accredited NP program.
• TX license and NP certification required.
• 1–2 years palliative care experience required.
• Inpatient setting experience preferred for transitions of care.

Responsibilities

• Focus on the care and transition of patients in the inpatient setting.
• Provide comprehensive palliative care services to patients and families.
• Report to supervising physician/Medical Director and meet CME requirements.
• Maintain clear communication with care teams and families.
