Job summary

Qualifications

Responsibilities

• Provide direct clinical services in line with standard social work practice.
• Plan and conduct individual, family and/or group therapy as assigned.
• Conduct and interpret Comprehensive Integrated Assessments.
• Review multidisciplinary assessments and contribute to treatment planning.
• Perform case management and discharge planning as directed.
• Attend treatment team meetings and summarize progress toward goals.
