Qualifications

• Proficiency with MS Office (Outlook, Word, Excel, PowerPoint).

Responsibilities

• Provide inpatient oncology case management for a group of patients.
• Assess discharge-planning needs at admission and throughout stay.
• Review patient progress and arrange services for safe transitions to next level of care.
• Coordinate home care, equipment, services, and follow-up care.
• Collaborate with physicians, nurses, social workers, and billing to obtain authorizations and support appeals.
• Follow select patients in the outpatient setting.
