Qualifications

• Requires Master’s Degree (MSW) from an accredited School of Social Work.
• Must sit for the LCSW Certification exam within 90 days of hire and obtain within 6 months.

Responsibilities

• Completes biopsychosocial assessments.
• Provides brief, solution-focused interventions and safety plans.
• Delivers trauma-informed, crisis interventions for patients and families.
• Develops rapport and maintains therapeutic relationships with patients and families.
• Uses motivational interviewing and trauma-focused CBT techniques.
• Assists patients and families to cope with illness and maintain functioning.
• Ensures weekly contacts and timely updates in care plans.
• Communicates with multidisciplinary teams and community providers.
• Screens for social determinants of health and safety concerns.
