Qualifications

• Master's degree in social work from an accredited CSWE program.
• 1 year of SW experience in any healthcare setting.
• 1 year of experience in acute care setting or completion of TIPs/Case Management Orientation Program.

Responsibilities

• Provide professional, comprehensive patient-centric care management in an acute care environment.
• Assess and plan, coordinate care, discharge planning, and document interventions.
• Ensure regulatory compliance and advocate for patients to address barriers to access.
