Qualifications

• Master's Degree in Social Work from an accredited CSWE program.
• One year of social work experience in any healthcare setting.
• One year of experience in acute care or completion of TIPs/Case Management Orientation.

Responsabilités

• Assess and plan patient care; develop care plans and coordinate related services.
• Coordinate care across disciplines and with discharge planning.
• Document interventions and ensure regulatory compliance.
• Advocate for patients and identify barriers to care access.
