You will interview patients, families, and caregivers, review pharmacy records, and document results in the EHR for pharmacist review.

The role requires Alaska licensure and a pharmacy technician certification, with 2 years hospital or clinic experience and strong communication and computer skills.

Qualifications

• Requires Alaska pharmacy technician licensure at hire.
• Pharmacy technician certification required (CPhT or ExCPT).
• Two years of hospital/ambulatory pharmacy technician experience or equivalent.

Responsibilities

• Obtain complete medication histories through structured patient interviews with patients, families, and caregivers.
• Verify information using pharmacy records and other approved sources and document sources.
• Document histories in the EHR within established timeframes and note completeness status.
• Identify discrepancies and refer to the pharmacist; no clinical interpretation provided.
• Coordinate medication history queues with pharmacy staff and external services.
• Participate in quality reviews and staff training related to medication histories.
• Protect patient privacy and maintain accurate documentation.
