Job summary

Qualifications

• LPN license issued by the state where services are provided.
• BLS certification within 30 days of hire date.

Responsibilities

• Provide direct patient care per plan of care and supervise role guidance.
• Perform therapeutic procedures as needed for assigned patients.
• Observe, document and report patient status and changes promptly.
• Assist physicians and other staff with examinations and procedures.
• Contribute to nursing assessment with data collection and reporting.
• Monitor patient condition and report deviations to the supervising nurse.
• Support development of the plan of care and implement care measures.
• Educate patients and families and participate in rehabilitation planning.
• Document nursing interventions and responses in the medical record.
• Administer medications via IM, subcutaneous, IV, oral, nebulization or MDI as assigned.
• Seek supervision when implementing new techniques or practices.
