Job summary

Live Well Homecare Agency, LLC is seeking an Inpatient RN CAP 2 to provide clinically competent care for dialysis patients in a hospital setting. The role involves assessment, planning, intervention, and evaluation, with opportunities to mentor staff and participate in quality improvement initiatives.

The position requires RN licensure and CPR/BLS certification, 2+ years nephrology experience (acute dialysis preferred), and the ability to lift up to 200 lbs.

Qualifications

• RN license and CPR/BLS certification required.
• 2+ years nephrology nursing experience; acute dialysis background preferred.
• At least 6 months of acute dialysis experience for supervisory oversight.
• Ability to perform heavy lifting and assist patient mobility in a clinical setting.
• Strong communication, teamwork and leadership skills are essential.

Responsibilities

• Perform patient assessments and document in medical records, adjusting treatment plans as needed.
• Educate patients and families on care needs and recognize deviations from normal patient status.
• Administer treatments, medications, blood transfusions, and respond to emergencies.
• Coordinate communication among healthcare providers to ensure continuity of care.
• Serve as a resource, mentor, and role model for staff and may assist in specialized care or outpatient settings.
• Complete CAP requirements for professional development and may supervise other staff as designated.
