Job summary

Carilion Clinic in Roanoke, VA seeks an RN Clinical Coordinator for Inpatient Dialysis to lead and coordinate patient care across shifts. You will oversee delivery of nursing care, mentor staff, and ensure alignment with clinical guidelines.

The role requires at least 3 years of RN experience, VA RN licensure, BLS-HCP certification (or obtained within 90 days), and ongoing specialty credentials. You’ll collaborate with physicians, social work, and other teams to improve outcomes and ensure

Qualifications

• Graduate of an accredited School of Nursing required.
• Virginia RN licensure required; BLS-HCP certification required or obtained within 90 days of hire.
• Minimum of 3 years RN experience.
• Additional certifications (ACLS/CCR N/ONC/Pain Management or other) may be required by department.
• Valid driver’s license for roles involving travel, such as Home Health.
• Strong interpersonal, communication, and teamwork skills.
• De-escalation training must be completed within one year of hire and maintained thereafter.

Responsibilities

• Provide direct and indirect patient care as needed; oversee delivery of patient care; and manage programs to coordinate care.
• Coordinate interdisciplinary care plans with physicians, nursing and ancillary staff.
• Develop, analyze, and evaluate clinical and operational data to support quality initiatives.
• Ensure compliance with regulatory and accreditation requirements (The Joint Commission).
• Educate patients, families and staff to support understanding of care plans and post-discharge needs.
• Advocate for patients and families and facilitate communication across care teams.
