Job summary

Carilion Clinic is seeking an RN Clinical Coordinator for Inpatient Dialysis in Roanoke, VA. This full-time role requires at least 3 years of RN experience, Virginia licensure, and BLS-HCP certification with ACLS/CCRN consideration based on the ward.

You will oversee the delivery of safe, high-quality patient care, coordinate interdisciplinary plans, and lead performance improvement initiatives while ensuring regulatory compliance and effective care transitions.

Qualifications

• Graduate of an accredited School of Nursing required.
• Bachelors degree preferred.
• Current RN licensure in the State of Virginia required.
• AHA BLS-HCP certification required or must be obtained within 90 days of hire.
• ACLS/CCRN or other specialty certifications may be required based on department.
• Valid driver’s license for roles involving travel.

Responsibilities

• Utilizes advanced nursing processes in clinical decision-making and care coordination to improve patient outcomes.
• Provides direct and indirect patient care as needed and oversees delivery of care within the department.
• Coordinates interdisciplinary care plans with physicians, nursing staff, social work, and ancillary services.
• Performs patient assessments, facilitates referrals, and ensures transitions across the continuum of care.
• Identifies barriers to care and implements strategies to improve outcomes and resource utilization.
• Develops and evaluates clinical and operational data to support quality improvement initiatives.
• Ensures compliance with regulatory standards (TJC, Medicare/Medicaid).
• Provides education to patients, families, and staff on care plans and post-discharge needs.
