Job summary

Cognizant is seeking an Inpatient Coder (CCS) to review medical documentation and assign ICD-10-CM/ICD-10-PCS codes for Cognizant clients in a remote, work-from-home environment. The ideal candidate has 3+ years facility-based inpatient coding, Level 1 Trauma experience, and Epic proficiency.

You will query physicians, perform peer reviews, and support coding quality while maintaining high accuracy and productivity in a fast-paced remote setting.

Qualifications

• Three years of facility-based inpatient coding experience (ICD-10-CM, ICD-10-PCS, MS-DRG).
• Level 1 Trauma coding experience required.
• Epic experience required.
• Knowledge of federal, state, and payer regulations.
• Ability to interpret medical records, reports, and chart entries.
• Ability to apply judgment and make decisions.
• Ability to work independently and as a team member, and to stay organized across multiple projects.
• Strong oral and written communication skills.
• Flexibility and adaptability to change.
• Proficiency in MS Office (Word, Excel, Outlook) and basic computer skills.

Responsibilities

• Review documentation to code diagnoses (ICD-10-CM) and procedures (ICD-10-PCS).
• Communicate with physicians to obtain or clarify diagnoses and/or procedures through the query process.
• Assign accurate codes using an encoder application in accordance with policy and regulatory guidelines.
• Perform peer reviews of coding auditors and coders.
• Complete reports as requested.
• Maintain a minimum accuracy rate of 98%.
• Meet inpatient productivity: 16 encounters per day, 2 per hour, 30 minutes to code each encounter.
• Work in multiple systems at the same time.
• Coordinate with your manager and team on time off and new policies.
• Perform other duties as assigned.

Job Summary

The Inpatient Coder is a key role supporting one or more Cognizant clients, responsible for reviewing medical documentation and accurately assigning ICD-10-CM diagnosis and ICD-10-PCS procedure codes. This role works independently and as part of a team, queries physicians to clarify documentation, and supports coding quality through peer review, while meeting accuracy and productivity standards in a fast-paced remote environment.

Key Responsibilities

• Review documentation to code diagnoses (ICD-10-CM) and procedures (ICD-10-PCS).
• Communicate with physicians to obtain or clarify diagnoses and/or procedures through the query process.
• Assign accurate codes using an electronic encoder application in accordance with practice policy and regulatory guidelines.
• Perform peer reviews of company Health Information Management coding auditors and coders.
• Complete reports as requested.
• Maintain a minimum accuracy rate of 98%.
• Meet inpatient productivity expectations: 16 encounters per day, 2 encounters per hour, and 30 minutes to code each encounter.
• Work in multiple systems at the same time.
• Coordinate with your manager and team on scheduled time off, breaks, and new procedures and policies provided by the client.
• Perform other duties as assigned.

Qualifications

• Coding credential: CCS, RHIT or RHIA (AHIMA), or CPC or CIC (AAPC).
• Minimum of three years of facility-based inpatient experience (ICD-10-CM, ICD-10-PCS, MS-DRG).
• Level 1 Trauma coding experience required.
• Epic experience required.
• Knowledge of federal, state, and payer regulations.
• Ability to interpret medical records, reports, and chart entries.
• Ability to apply judgment and make decisions.

• Active CCS, RHIT, RHIA, CPC, or CIC credential

• Epic experience
• Pass the coding assessment with a score of 80% or higher

Compensation

The annual salary for this position is between $63,000 – $71,000, depending on experience and other qualifications of the successful candidate.

This position is also eligible for Cognizant’s discretionary annual incentive program, based on performance and subject to the terms of Cognizant’s applicable plans.
