Job summary

Crossing Hurdles is seeking an experienced inpatient coder to review medical records and assign ICD-10-CM/PCS codes with a focus on accuracy and coding integrity.

You will validate MS-DRG assignments, flag hospital-acquired conditions and POA indicators, and work with clinical documentation improvement and AI-generated notes to ensure compliant documentation.

Qualifications

• Active CIC, CCS, or AHIMA/AAPC coding certification.
• Experience in inpatient ICD-10-CM/PCS coding.
• Access to up-to-date coding references and Coding Clinic guidance.
• Currently working in inpatient medical coding or clinical documentation improvement.

Responsibilities

• Review and analyze inpatient medical records for accurate ICD-10-CM and ICD-10-PCS coding.
• Review clinical documentation for accuracy, completeness, and coding integrity.
• Identify principal and secondary diagnoses, sequence codes, and present on admission (POA) indicators.
• Validate MS-DRG assignment and flag hospital-acquired conditions and patient safety indicators.
• Evaluate AI-generated clinical notes against real-world documentation and coding standards.
• Identify and resolve discrepancies in diagnosis and procedure coding.
