Job summary

Forest View Hospital, part of Universal Health Services, seeks an Inpatient Clinician to provide psychiatric case-management and therapy across diverse patient needs in Grand Rapids, MI. You will coordinate care with hospital teams, community resources, and discharge planning while maintaining documentation per standards.

The role requires Michigan licensure (LMSW/LLMSW/LLPC/LPC/LLP/TLLP) and experience in group therapy is preferred. Fulltime day shift with competitive benefits is offered by UHS.

Qualifications

• Previous group therapy experience preferred but not required.

Responsibilities

• Completes psychosocial assessments and collaborates with patients to set individualized treatment goals.
• Initiate discharge planning and arrange aftercare appointments/referrals.
• Coordinate treatment interventions and update plans every 7 days or as needed.
• Conduct group therapy and other groups as assigned.
• Facilitate individual and/or family sessions as needed.
• Document interactions per hospital standards.
• Attend required team meetings.

Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for an Inpatient Clinician.

The Inpatient Clinician provides psychiatric case-management and therapeutic services to assigned patients with diverse needs to ensure there is a coordination of care between the hospital, community, treatment, therapeutic services, education and discharge planning.

Essential Job Responsibilities:

• Completes a thorough psychosocial assessment and collaborates with the patient to develop individualized treatment goals.
• Initiate discharge planning and arrange aftercare appointments/referrals.
• Oversee the treatment planning process with person-centered planning and will coordinate all disciplines in treatment interventions, updating treatment plans every seven (7) days or as needed.
• Conducts group therapy and other groups as assigned.
• Facilitate individual and/or family sessions as needed.
• Documents each interaction with patient that is consistent with hospital standards.
• Attends all appropriate team meetings as required
• Other duties as assigned.

Hours: Fulltime - Day Shift.
