Job summary

Qualifications

• Education: MSW from an accredited program.
• Licenses/Certifications: Texas LMSW; ACM certification preferred.
• Experience/Knowledge/Skills: health services experience; crisis intervention; DSM-V/ ICD-10 knowledge.

Responsibilities

• Assess psychosocial risk factors and support systems.
• Develop and carry out multidimensional discharge plans.
• Coordinate with care team for post-discharge resources.
• Provide crisis intervention and mental health assessments.
• Educate patients/families on post-acute options and resources.
• Advocate for patient empowerment and independence.
