Qualifications

• Pharmacist with a bachelor’s degree and active license.
• Minimum 1 year of related experience in clinical pharmacy.

Responsibilities

• Directs and coordinates clinical activities in the assigned pharmacy service area.
• 2. Reviews physician orders for appropriateness, allergies, interactions, and dosing.
• 3. Communicates with physicians on orders and intervenes as needed.
• 4. Ensures proper medication preparation, labeling, and staff supervision.
• 5. Provides drug information using pharmacology knowledge and literature.
• 6. May conduct medication history interviews and reconcile therapies.
• 7. Anticipates controlled drug needs based on disposition records.
• 8. Completes medication station inspections with assistants.
• 9. Participates in medication education and documentation.
• 10. Performs order entry and pharmacist functions in the Pharmacy system.
• 11. Participates in multidisciplinary meetings and discharge planning to optimize reconciliation.
• 12. May conduct home visits to review medications and educate patients.
• 13. Engages in Pharmacy Quality Programs and safety initiatives.
• 14. Provides operational support and coverage as needed (weekends/evenings/holidays).
• 15. Provides continuing education and serves as preceptor.
• 16. Participates in development of treatment guidelines and pathways.
• 17. Contributes to department initiatives on drug-specific and population-specific programs.

Bachelor's degree in Pharmacy
