Tufts Medicine is seeking a Clinical Pharmacist to lead clinical activities, review medication orders, and provide drug information to patients and staff in a hospital setting.

Ideal candidates have a Pharmacy license, a Doctor of Pharmacy (PharmD) degree preferred, and strong communication skills to collaborate with physicians, nurses, and other health professionals. The role includes education and participation in medication safety and quality programs.

Qualificações

• Bachelor’s degree in Pharmacy required.
• Pharmacist License required.
• Minimum of 1 year related experience.
• Doctor of Pharmacy (PharmD) preferred.
• Board Certification preferred.
• Four years of related experience in clinical pharmacy preferred.

Responsabilidades

• Directs and coordinates the clinical activities of the assigned pharmacy service area.
• Prepare, compound and dispense medications in accordance with state and federal laws.
• Review physician orders for indication, dose, allergies and interactions.
• Communicate with physicians about medication orders when needed.
• Provide drug information and education to patients and families.
• Perform medication history interviews and reconciliation.
• Assist in medication use evaluation and optimization of therapies.
• Participate in multidisciplinary team meetings and discharge planning.
• Supervise support staff on shift and ensure regulatory compliance.
